What to Expect for Your First Visit

We’ll request your credit card information to schedule your appointment and will bill you when you check in.

You’ll need your insurance card for any prior authorizations for medications/treatments. We don’t bill your insurance for visits.

 

 

How should I prepare for my first visit?

Make sure to complete all paperwork and intake forms before your scheduled appointment. Please email, fax, or bring the following with you:

  • Medical records from other treating physicians.

  • A list of all medications, including dosages and directions (supplements, herbs, and OTC medications included).

  • Diagnostic testing results (lab results, brain MRI, CT scans, etc). If you have a disc with images, please bring it to your appointment.

Policies

Cancellation Policy

We understand circumstances may change but our office needs to be informed in advance of any cancellations.  Patients will be charged the full amount of the visit unless they cancel at least 1 business day (24 hours or more, excluding weekends and holidays) in advance.  Cancellation or rescheduling requests submitted less than one business day in advance of the appointment will result in a charge of the full visit fee.

Late Policy

Patients arriving late will only be seen during their regularly scheduled appointment time and may have their appointment times cut short. If you arrive too late you may be asked to reschedule. Late patients are charged the full fee for their scheduled service.

No Show

Patients who do not show for their scheduled appointment will be charged the full amount of the visit unless they cancel 24 hours (excluding weekends and holidays) or more in advance.

Prescription Refill Policy

You should generally have enough refills to hold you until your next appointment with us.  If you notice that you are running low on your medication, it likely means that you need to schedule an appointment. All patients are required to have an appointment with us – either in person or by telemedicine – at a minimum of every 6 months. After 1 year of not being seen, patients are considered inactive, and prescriptions will not be refilled.

Texas Headache and Facial Pain Center does not participate with any insurance plans. Payment is due in full at the time of service. Pricing for visits and procedures are listed below. After each visit, we will provide you with a “superbill” with the correct CPT and diagnosis codes. These can be submitted by you to your insurance company for potential reimbursement, depending on your out-of-network benefits.

Initial Consultation:  $625

Follow-Up Visit:  $325

 

Procedural Charges

Therapeutic Botox:  $300 (Note:  This does not include cost of the drug)

Peripheral Nerve Blocks:  $350 (global fee, both sides) or $50 per injection

Trigger Point Injections:  $100 per side

SPG Blocks:  $50 per side

Prior Authorization for a Medication: $50 per medication, per year

Frequently Asked Questions

 

Do you take insurance?

Texas Headache and Facial Pain Center does not participate with insurance plans. An itemized statement and receipt of payment will be provided at the time of the encounter that you may submit to your insurance plan for reimbursement, depending on your out-of-network benefits.  Note that you cannot submit for reimbursement to government payers such as Medicare, Medicaid or Tricare.

Why don’t you take my insurance?

Today, there are many health insurance companies, each offering a wide range of plans. These plans come with different rules, restrictions, and payment structures for physicians. Unfortunately, insurance reimbursement rates are steadily declining across all types of medical care. Doctors within insurance networks are often required to accept significantly lower fees than in the past, even as the cost of running a medical practice continues to rise.

In addition, dealing with insurance companies creates substantial administrative burdens. Physicians must spend more time on paperwork and rely on larger office staff to handle the bureaucracy. This leaves less time for patient care and can impact the overall quality of treatment.

At Texas Headache and Facial Pain Center, we take a different approach. We focus on providing high-quality, personalized care—especially for patients with complex medical histories who may have already seen other doctors. By allowing more time for each appointment, we can better understand each patient’s needs and goals. Our model frees us from administrative distractions, enabling us to prioritize your care and experience. 

 

If you do not participate with insurance, why do you need my insurance information?

We need your insurance information for prior authorizations for medications and procedures and ordering tests.

 

 

What if I have out-of-network benefits?

If you have out-of-network benefits, you can submit our charges to your plan for reimbursement for your out-of-pocket expense. Factors like deductibles and cost-sharing may apply.  Many plans require that you must first meet a deductible amount (whether in-network or out-of-network). This means that a certain amount of the claim payment will be withheld in order to meet the deductible amount within a specified timeframe. Patients with out-of-network benefits can expect to be reimbursed for some or most of the out of pocket expenses, typically 50-80%, once their deductible is met.  Additionally, some plans include a coinsurance feature, which requires the member to contribute a percentage of the total payment for services, in addition to the deductible. Your insurer will process your claim, determine if there is a cost-share amount, and reimburse you for any amount due to you from there.

I need an MRI and blood tests. Are those covered?

Yes. All outside testing gets billed through your insurance

​

Botox injections can be expensive. Will I be responsible for the cost of the drug?

In most cases, we can get injectable medications through your insurance, and the Botox Savings Program can assist with the out-of-pocket expenses. You will only be charged for the injection fee ($300).

Do you prescribe narcotic medications?

No.  Texas Headache and Facial Pain Center operates as a non-narcotic headache center.  Studies show that routine use of narcotic medications, including opioids and barbiturates, for migraine treatment often leads to more frequent and severe headaches, and significantly increases the likelihood of developing chronic headaches.  They may also decrease the effectiveness of headache treatments. There is also very little evidence for effectiveness of opioids for facial pain.
